Herriot Hospice Homecare services are accessed via a referral from a healthcare professional involved in your care, unless stated. Our list of services below outlines referral information and forms. Please only use these forms if you are referring a patient to Herriot.
At Herriot Hospice Homecare, we know that often people wish to receive care and support in the place they call home, in familiar surroundings with family, friends and the things that matter to them.
Our expert healthcare assistants visit people in their own homes to provide care in various ways. This could be personal care, support with toileting, washing, mouthcare, dressing, positional changes and bedding changes, as well as supporting the patient and their family with their emotional wellbeing.
Referrals to this service must be made by a professional, and this is done over the phone by calling our colleagues at South Tees End of Life Coordination Team on (01609) 764 440, seven days a week, 8.30am-8.30pm.
Once our inpatient hospice care services are operational from Herriot Hospice at the Lambert, a referral form will be available here.
Our Community Outreach Service offers practical, emotional, social and spiritual support, tailored to a person’s individual needs.
The support available includes outpatient clinics to help people affected by terminal illness to manage symptoms, as well as accessing peer support groups and professional advice.
Referrals can be made for patients, families, and carers over the age of 18, affected by terminal illness.

Current services include 1:1 emotional support, our drop-in, and support from our wellbeing volunteers.
Our weekly drop-in service at Herriot Hospice at the Lambert requires no referral.
If you’re affected by terminal illness, bereavement or require emotional wellbeing support, as part of our Community Outreach offer, the Just ‘B’ team invite you to drop in every Tuesday 2-4pm at the Lambert, to access information and meet other for peer support over a cup of tea.
Volunteer Visitors
Herriot’s befriending and sitting volunteer-led services provide reliable social and emotional support to people in their own homes. Volunteer support can range from help with simply being there and listening to supporting carers to get that valuable break.
Referrals can be made for patients over the age of 18, with a palliative diagnosis and expected to be in the last 6 months of life.

This service has been paused and will no longer be taking further referrals.
This service launched thanks to the generous support of Humber and North Yorkshire Cancer Alliance, and we are now accepting referrals.
An outpatient clinic for people living with a terminal illness and experiencing breathlessness. Our sessions will offer practical exercises, advice and support to manage symptoms and improve quality of life.
The clinic will usually be delivered over six outpatient appointments. Over this time, our doctor, occupational therapist, physiotherapist and our wellbeing team work together, bringing their unique expertise to your care. We may also discuss referring you to other professionals who we think could help you.
Appointments will take place at Herriot Hospice at the Lambert in Thirsk. While CQC authorisation is pending for regulated care services to be provided from the space, we will temporarily deliver sessions in patients’ homes. Home visits and telephone support may continue to be offered later if clinically appropriate.

Just ‘B’ is part of Herriot’s family of services and provides pre and post bereavement and emotional wellbeing support to children, young people and adults across North Yorkshire. Just ‘B’ is accessed by both self-referral and professional referral. Find out more on the Just ‘B’ website.
