Referrals

Our care is available to people living in Hambleton and Richmondshire. All our services are free for patients and their families or carers. We provide care for people aged 18-years-old and upwards.

If you think you could benefit from using a Herriot service, please talk to your GP, district nurse, specialist nurse, consultant or any other healthcare professional involved in your care about your options.

Accessing our services and information for professional referrers

Herriot Hospice Homecare services are accessed via a referral from a healthcare professional involved in your care. Our list of services below outlines referral information and forms. Please only use these forms if you are referring a patient to Herriot.

HOME service

At Herriot Hospice Homecare, we know that often people wish to receive care and support in the place they call home, in familiar surroundings with family, friends and the things that matter to them.

Our expert healthcare assistants visit people in their own homes to provide care in various ways. This could be personal care, support with toileting, washing, mouthcare, dressing, positional changes and bedding changes, as well as supporting the patient and their family with their emotional wellbeing.

Referrals to this service must be made by a professional, and this is done over the phone by calling our colleagues at South Tees End of Life Coordination Team on (01609) 764 440, seven days a week, 8.30am-8.30pm.

Inpatient care

Once our inpatient hospice care services are operational from Herriot Hospice@The Lambert, a referral form will be available here.

Herriot Wellbeing Network 

Our Wellbeing Network offers physical, practical, emotional, social and spiritual support, tailored to a person’s individual needs.

The support available includes outpatient clinics to help people affected by terminal illness to manage symptoms, as well as accessing peer support groups and professional advice.

Referrals can be made for patients, families, and carers over the age of 18, affected by terminal illness.

We also provide a weekly drop-in service as part of our Wellbeing Network, which requires no referral. 

Referrals can be made via SystmOne, or through our referral form. Please return this form to wellbeing@herriothh.org.uk or call (01423) 878 189 Please ensure the referral form is emailed as password protected, or via Egress. 

Current services include 1:1 emotional support, our drop-in, and befriending, driving and sitting services. Our Share&Support group to come later in 2024, once Herriot Hospice@The Lambert is open. Share&Support will be a new peer support group for people living with terminal illness, their families and carers, to come together to provide each other with social and emotional support in a relaxed and welcoming environment.

Herriot Care Connector 

Herriot Hospice Homecare is available to provide practical support for people affected by a palliative diagnosis or living with long-term medical conditions.

The charity’s Care Connector is available across Hambleton and Richmondshire and is also supporting those that find themselves socially isolated due to their circumstances or rurality.

Patients do not need to be living with a terminal illness or already accessing Herriot Hospice Homecare services to access this support.

Referrals can be made by healthcare professionals by completing our referral form. Hambleton and Richmondshire GPs can also refer via SystmOne.

Breathing Space

This service is launching in December 2024 thanks to the generous support of Humber and North Yorkshire Cancer Alliance, and we are now accepting referrals.

An outpatient clinic for people living with a terminal illness and experiencing breathlessness. Our sessions will offer practical exercises, advice and support to manage symptoms and improve quality of life.

The clinic will usually be delivered over six outpatient appointments. Over this time, our doctor, occupational therapist, physiotherapist and our wellbeing team work together, bringing their unique expertise to your care. We may also discuss referring you to other professionals who we think could help you.

Appointments will take place at Herriot Hospice@The Lambert in Thirsk. While CQC authorisation is pending for regulated care services to be provided from the space, we will temporarily deliver sessions in patients’ homes. Home visits and telephone support may continue to be offered later if clinically appropriate.

Referral criteria are:

  1. Breathlessness symptoms (in the context of advanced, terminal disease) which persist/progress despite optimised management of underlying condition.
  2. Patients and their carers can engage cognitively and functionally with self-management programmes, given an appropriate level of support with access.
  3. Patient is resident in Hambleton and Richmondshire and registered with a GP in this area.

Health professionals can refer patients by completing our referral form. Hambleton and Richmondshire GPs can also refer via SystmOne.

Digital support service

Patients living with terminal illness and their carers are welcome to drop-in for free practical help with online tasks. Our trained digital champions can help patients to:

  • Book an appointment online
  • Log on to a video call
  • Manage health information and records online
  • Keep in touch with friends and relatives
  • Do anything else digitally and you’d like to ask questions

Our sessions take place every Wednesday 10am-2pm at Herriot Hospice@The Lambert – see event.

Our digital support page also hosts a handy directory of places to turn for support. From mental health and eating disorders, to funeral planning and financial advice.

Referrals are not required for this service, but we encourage you to recommend to patients and families.

Just ‘B’

Just ‘B’ is part of Herriot’s family of services and provides bereavement and emotional wellbeing support to children, young people and adults across North Yorkshire. Just ‘B’ is accessed by both self-referral and professional referral. Find out more on the Just ‘B’ website.
