End of Life Care
As part of Herriot Hospice Homecare’s end of life care service, trained Herriot healthcare assistants visit people in their own homes to offer support with personal care such as bathing and the monitoring and management of symptoms, together with other community professionals.
We know that many people wish to die at home, surrounded by the people and things that matter most, and the service helps people to stay in their preferred place of care for as long as possible.
This care is personalised and responsive to the needs of the individual, helping them to feel more comfortable, prevent unnecessary admissions to hospital and enabling people to return home from hospital sooner.
The service is majority funded by North Yorkshire NHS Clinical Commissioning Group (CCG), and can therefore be accessed by patients assessed for fast track funding, as part of a wider package of care delivered by healthcare professionals from various local organisations.
