Practical support from Herriot Hospice Homecare

Herriot Hospice Homecare is available to provide practical support for people affected by a palliative diagnosis or living with long-term medical conditions.

The charity’s Care Connector is available across Hambleton and Richmondshire and is also supporting those that find themselves socially isolated due to their circumstances or rurality.

You do not need to be living with a terminal illness or already accessing Herriot Hospice Homecare services to access this support.

You will be supported for up to 12 weeks, from Herriot Hospice@The Lambert, at home or over the phone, with a range of support, including:

  • 1:1 support and goal setting.
  • Financial signposting, including applying for person-specific grants, support in completing forms and budgeting support – as well as signposting to specialist organisations.
  • Social isolation support, including transport to and interaction with local groups, art or music therapy at home if unable to travel (i.e. for those with a palliative diagnosis), and establishing the reason for social isolation.
  • Provide support following bereavement and enable clients to join a local bereavement group.

The service will be bespoke, tailored to your individual needs and wishes following an initial discussion with the Care Connector. It is part of our family of services’ commitment to reaching 50% more people by 2027.
