An update on our Care Connector service

Our Care Connector service – which has providing practical support for people affected by a palliative diagnosis or living with long-term medical conditions across Hambleton and Richmondshire for the last year – has been paused and will not be taking further referrals.

This difficult decision comes as part of a review into our services, ensuring they are effectively and sustainably meeting the needs of those we serve.

Our wider range of services continue to be here for people, families, friends and carers living with terminal illness when they need us most. We do this through our end of life care in the place people call home, our wellbeing services, our Community Hub, wider outpatient services, Just ‘B’ bereavement support, and upcoming inpatient care from Herriot Hospice at the Lambert.
