Our HOME service

Referrals to our HOME service come via healthcare professional in your community.

Bereavement support via Just ‘B’, part of our family of services

Adults referring into our bereavement services can be referred by a professional or they can self-refer. If they are referred by a professional, the person must consent to the referral.

Children and young people’s bereavement services are available to young people under the age of 18 and they can be referred to this service by a parent/carer or an appropriate agency they are in contact with. Children and young people’s bereavement referrals can also be made by schools.

Wellbeing Network

Our weekly drop-in at Thirsk Town Hall is accessible to anybody, with no referral needed. Share&Support is accessed via healthcare professional only.

For referrers

Referrals to our HOME service can be made by SystmOne. For enquiries on referrals into the HOME service, please call our on call number 07976 324 761, available between 7am and 11pm every day.

Referrals to Just ‘B’ can be made directly via the above.

If you are a health or social care professional wishing to refer someone to Herriot’s Share & Support group, part of our Wellbeing Network, please call our Wellbeing Team on (01423) 878 189 to make the referral.
